When We Accept Your Application, Please Be Sure That:

 

1.         The passport application is complete but not signed:

• mailing address is where the passport will be sent

•  social security number omission could result in an IRS fine

• permanent address is street, apt #, RFD, city, state, zip code...but not a P.O. Box

• parents' data is needed even if they are deceased - approximate dates may be used

• travel plans or “none” must be entered

• approximate issue date of previous passport is supplied;  A "lost passport statement" (DS-64) is required if the passport is still valid and not submitted

• marriage information is complete...all information refers to most recent marriage

• in case of emergency notify someone in U.S. not traveling with applicant

         2.         The applicant submits proof of U.S. citizenship (see attached)

 3.         The applicant submits 2 recent identical photos (see attached)

 4.         The applicant or minor applicant's parents present ID:  (see attached)

 5.         The agent administers oath and witnesses your signature:

·                  all applicants must appear in person

·                  both parents must also appear in person and sign on behalf of minor under 16

(see attached for more info on minors under 16)

·                  one parent must also appear in person and countersign for minor 16 and 17

6.         Appropriate fees are submitted:

For applicant 16 or older -- $100.00 for passport book; $45 for passport card

·        Check or money order payable to Department of State:

            $75.00 for passport book, or

            $20 for passport card

·        AND Cash, Check or money order payable to Town of Brunswick: $25.00

(If applying for both only one $25 fee is applicable)

For applicant under age 16 -- $85.00 for passport book; $35 for passport card

·        Check/ money order payable to Department of State:

                                    $60.00 for passport book, or

                                    $10 for passport card

·        AND Cash, Check or money order payable to Town of Brunswick: $25.00

(If applying for both only one $25 fee is applicable)

 Passport card is not for travel by air.  It is valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only

 

EXPEDITE FEE:  ADDITIONAL $60 PER APPLICANT

The Expedite fee is $60.00 in addition to the cost of the passport.

 Expedite processing times fluctuate with volume; generally about three weeks.

 You may also pay for Express Mail overnight postage at the passport acceptance facility.

 If traveling or needing a visa in less than 14 days:  you must apply directly to the Passport Agency.  (Call 617-878-0900 for an appointment with the Boston Passport Agency.)  Note:  you will need proof of imminent departure, such as tickets or itinerary.

You may also contact a private passport processing service to hand carry your executed application to a Passport Agency for an additional fee.
