Passports

PASSPORT APPLICATIONS ARE ACCEPTED 8:30 A.M. TO 4:00 P.M. MONDAY THROUGH FRIDAY

OUR OFFICE DOES TAKE PASSPORT PHOTOS - $12

All applicants must appear in person. 

Minors:

  • Under Age 16.  Both parents must appear in person and sign on behalf of a minor under the age of sixteen.

  • Ages 16 and 17.  One parent must appear in person and countersign the minor's application. 

 

When You Submit Your Application Please Be Sure To:

  1.  Complete the application using BLACK ink.  DO NOT SIGN the application in advance.  The agent must witness your signature.

  2.  Submit proof of U.S. citizenship (Click for more information)

  3. Submit one recent photo (Click for more information)

  4. Submit acceptable identification for the applicant or minor applicant's parents.  (Click for more information)

  5. Submit Fees.  Both Passport and Execution Fees are require for each application.

Passport Fees - Check or money order made payable to Department of State. (No cash; no credit or debit cards)

  •  Applicants age 16 and older -- $110 for passport book; $30 for passport card.  Check or money order payable to Department of State.

  • Applicants under age 16 -- $80.00 for passport book; $15 for passport card.  Check or money order payable to Department of State.

  • Expedite Fees - $60 per applicant. OPTIONAL.  (Click for more information about expediting the process)

Execution Fees - Check or money order made payable to the Town of Brunswick. Also cash, credit or debit card.

  • Execution Fee  - $25 per application regardless of whether you are applying for passport book, passbook card, or both.

 Frequently Asked Questions about the application:

  1. Mailing Address - where the passport will be mailed to applicant.

  2.  Permanent Address - where the applicant's.  It is the street, apt #, RFD, city, state, zip code...but not a P.O. Box.

  3.  Parental Information - required even if parent(s) are deceased.

  4.  Travel Plans - approximate dates may be used.

  5.  Approximate Issue Date of Previous Passport Book or Card -  A "lost passport statement" (DS-64) is required if the passport is still valid and not submitted.

  6.  Marriage Information - all information refers to most recent marriage

  7. Emergency Notification - notify someone in U.S. not traveling with applicant
