PASSPORT APPLICATIONS ARE ACCEPTED 8:30 A.M. TO 4:00
P.M. MONDAY THROUGH FRIDAY

All applicants must appear in person.

advance. The agent must witness your signature.

Fees - Check or money order made payable to Department of State.
(No cash; no credit or debit cards)

-- $110 for passport book; $30 for passport card. Check or money
order payable to Department of State.

$80.00 for passport book; $15 for passport card. Check or money
order payable to Department of State.

passport will be mailed to applicant.

applicant's. It is the street, apt #, RFD, city, state, zip code...but
not a
P.O. Box.

Issue Date of Previous Passport Book or Card - A "lost passport statement"
(DS-64) is required if the passport is still valid and not submitted.
